Enrollment Options
Active clergy participating in the Clergy Pension Plan are automatically enrolled in the Long-Term Disability Benefit.
Employer-Provided Long-Term Disability Plan
If you provide Long-Term Disability Insurance as a benefit of employment, current employees are enrolled as of the date you adopted the plan.
You must enroll all eligible new employees within 31 days of their hire date. If you enroll employees after 31 days, your organization will be responsible for back premiums.

Benefit Reductions
Benefits provided under the long-term disability insurance policy are subject to restrictions and limitations.
Coverage Conversion
Long-Term Disability Insurance is portable, as long as the employee has been insured for at least 12 months. Any insured employee whose employment is terminated, and who wishes to continue coverage, may be able to convert the policy directly through Aflac. The Application for Conversion of Group Long-Term Disability Insurance must be received by Aflac within 30 days of the employee's termination date.

Voluntary Long-Term Disability Plan (Employee Paid)
If your institution does not provide long-term disability insurance as a paid benefit, you may offer an employee paid coverage option. Employees may purchase coverage for 50% of their covered compensation, up to $5,000 per month.

Portability
Once insured for at least 12 months, an employee whose employment is terminated may be eligible to continue coverage for up to 12 months by applying directly to Aflac. However, Aflac must receive the Application for Conversion of Group Long-Term Disability Insurance within 30 days of the employee's termination date.
